Tension headaches are associated with muscle tension in the neck or back, and misalignments of the spine in the neck causing pressure on the nerves.  Many patients report stiffness and decreased range-of-motion of the neck, and occasionally shoulder and arm pain.  Physical examination and x-rays can determine if there is disc degeneration, structural misalignment, or arthritis of the cervical spine (facet joints) which is a primary cause of tension headaches.

Evidence-Based Research

Research at the Duke University Evidence-Based Practice Center found that spinal manipulations resulted in almost immediate improvement for those headaches that originate in the neck and had significantly fewer side effects and longer-lasting relief of a tension headache than commonly prescribed medication.  Also, a study in the Journal of Manipulative and Physiological Therapeutics found that spinal manipulation therapy is an effective treatment for tension headaches and that those who ceased manipulation treatment after four weeks experienced a sustained therapeutic benefit in contrast with those patients who received a commonly prescribed medication.
